Vaping and Pack Years

Vaping does not convert into pack-years. Pack-years measure cigarette smoke, and all major screening rules (USPSTF, Medicare, American Cancer Society) are based on cigarettes only. Calculators that turn “puffs” or “pods” into cigarettes rely on nicotine content, but nicotine is not what causes most lung cancer — combustion smoke is. If you smoked cigarettes before switching, those years still count.

Why puffs and pods can’t be turned into pack-years

What to tell your doctor

Your historyHow it is recorded
Smoked cigarettes, then switched to vapingPack-years from the cigarette years only, plus vaping noted separately. Your “quit date” for screening rules is usually the date you stopped cigarettes.
Smoke and vape (dual use)Pack-years from cigarettes (keep counting them), vaping noted separately.
Only ever vapedNo pack-years. Record device type, nicotine strength and how long you have vaped.

Work out the cigarette part with the pack-year history calculator — add only the periods when you smoked cigarettes.

Vaping and lung cancer screening

Screening eligibility is decided by your cigarette pack-years, age and years since you quit cigarettes. Switching to e-cigarettes does not remove the risk built up from earlier smoking. If you had 20+ pack-years of cigarettes, check whether you qualify: screening after quitting.

E-cigarettes are not risk-free, and long-term effects are still being studied. If you want to stop nicotine altogether, free U.S. support is available at smokefree.gov or 1-800-QUIT-NOW.

Frequently Asked Questions

Can you calculate pack-years for vaping?

No. There is no validated conversion from vaping to pack-years. Pack-years are calculated from cigarettes only; vaping is recorded separately.

How many cigarettes is one vape pod?

Nicotine comparisons exist, but they vary widely by product and user, and nicotine content does not reflect cancer risk. Such figures should not be used to calculate pack-years.

I switched from cigarettes to vaping. Do my pack-years still count?

Yes. Your cigarette pack-years stay the same, and screening rules usually treat the date you stopped cigarettes as your quit date.

Does vaping make me eligible for lung cancer screening?

Not on its own. U.S. screening criteria are based on cigarette pack-years, age and years since quitting cigarettes.
