Cigars, Pipes and Roll-Your-Own: What Counts Toward Pack Years?

Pack-years are defined for cigarettes: (cigarettes per day ÷ 20) × years. Hand-rolled cigarettes count like any cigarette. For cigars, cigarillos and pipes there is no standard conversion; studies use different methods that are not comparable with cigarette pack-years. U.S. screening guidelines are based on cigarette pack-years only.

Why there is no standard cigar or pipe conversion

Practical approach: record cigarettes as pack-years, and record cigars, cigarillos and pipes separately as how many per day/week, for how many years, and whether you inhaled. Your doctor can weigh this when judging your overall risk.

Roll-your-own and pouch tobacco

Hand-rolled cigarettes are cigarettes, so count them directly: 15 roll-ups a day for 20 years is 15 ÷ 20 × 20 = 15 pack-years. If you only know how much loose tobacco you use, count how many cigarettes a pouch makes for you and work out the number per day — roll-up size varies a lot between smokers, so a fixed grams-per-cigarette rule can be misleading.

Screening if you smoked cigars or pipes

U.S. screening criteria (20+ pack-years, age 50–80) are based on cigarettes. If you smoked cigars or a pipe heavily — especially if you inhaled or also smoked cigarettes — tell your doctor; they may consider your overall risk. Cigarette years always count, so add them with the pack-year history calculator.

Frequently Asked Questions

How do you calculate pack-years for cigars?

There is no official method. Cigar size ranges from cigarette-like little cigars to large cigars containing as much tobacco as a pack of cigarettes, and research uses different conversions. Record cigars separately; screening rules count cigarettes only.

Do roll-your-own cigarettes count toward pack-years?

Yes. Hand-rolled cigarettes are counted like manufactured cigarettes: cigarettes per day divided by 20, multiplied by years smoked.

Is pipe smoking included in pack-years?

Not in the standard definition. Screening guidelines count cigarettes only; tell your doctor separately how much and how long you smoked a pipe.

Are cigars safer than cigarettes?

No. Cigar smoking causes cancers of the mouth, throat, oesophagus and lung. Risk is generally lower than for cigarette smokers who inhale, but rises with the amount smoked and with inhaling.
